Peninsula Health's At Home Remote Monitoring service assists patients with managing their health conditions at home.
The service is free and acts like an "early warning alert system" for patients with serious health conditions.
Peninsula Health is using all the iHealth wireless remote patient monitoring devices - TRACK blood pressure monitor, AIR pulse oximeter, PT3SBT Infrared Thermometer and NEXUS scales.
"If my health deteriorated and I didn't know what to do, the first thing would be to ring the ambulance. The ambulance comes and they see I'm in distress. I go into the ambulance and off to the hospital.
Now I ring the service, and then the doctor, the nurse, whoever, gets in touch with me, pretty much immediately and takes it from there. It's stopped me being admitted to hospital three times. Anybody that's offered the service is incredibly fortunate."
